Self Care Catalysts is a patient-centered digital health company that puts patients at the center and intersection of human networks, technology and science. We enable patients to become active and informed participants of their journey, while helping healthcare stakeholders optimize time to meet patient needs and generate real world evidence, in real time, longitudinally.

What products and services do you offer?
Our Health Storylines Platform is a trusted companion for patients throughout their journey, which generates real world evidence along the way. This includes real world data collection, patient monitoring, patient engagement and expanding opportunities for collaboration within and between stakeholder organizations. The platform enables real world, real-time, longitudinal data collection and patient support through the interaction between the Health Storylines patient-facing mobile app and practitioner-facing Health Storylines Creator.
Health Storylines supports both real-world patient research and care delivery through the integration of customizable self-care tools, EDC, eCRF, eCOA, ePRO, patient experience and preference data collection tools as well as passively collected data synced from wearable devices. Virtual health/telehealth capabilities can be used to follow up with research participants or treat patients remotely in conjunction with real-time remote monitoring. It also supports study set up, scheduling, monitoring and communications.
What problems does your organization solve?
Science and medicine’s roles are centered on diagnosis and treatment, often confined to the clinical setting, while the patient journey continues to unfold in homes, in the workplace and communities. With the ecosystem’s goal to improve healthcare outcomes, reduce cost of care delivery, accelerate drug development, generate evidence for regulatory and commercial purposes, the need to understand and interact with the patient holistically and longitudinally becomes more compelling.

What are the most innovative tools and technologies that you offer?
Our platform, Health Storylines is an end to end, digital-based disease-agnostic, customizable and personalizable patient journey platform that enables patient monitoring and surveillance, patient engagement, treatment planning, delivery of care intervention while generating real-world, real time, longitudinal patient data that inform decision making from treatment planning, clinical development, regulatory, market access, pre-commercial, post-marketing authorization decisions.
What are your competitive advantages?
Our platform, Health Storylines, connects the patients to an often siloed healthcare ecosystem, putting patients at the center of human networks, expanding technology and its applications and the growing sources of data science. We built our platform first and foremost with the patient in mind who is often lost in the system and follow them throughout their journey. Improving the flow of information throughout the healthcare ecosystem facilitates each stakeholder in achieving their objectives, ranging from improved outcomes, streamlined operations, reduced costs and collecting high-quality data for decision-making.
By centering the patient in the research and care process, unique and novel data related to comorbidities and health-related quality of life offer insight beyond disease-specific approaches that often target the requirements of only one stakeholder. The data generated through the platform can be used to understand individual patient journeys as well as achieve cohort level insights based on the Health Storylines Analytics dashboard that visualizes aggregated and de-identified data in real-time.
